The client understands and agrees to the following -

Nutritional therapy Is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and well being.

Practitioners do not recommend nutritional therapy as a replacement for medical advice and as such clients presenting with “red flag” signs or symptoms will be referred to a medical professional.

I am responsible for contacting my GP for any health concerns.

If I am receiving treatment from my GP, or any other medical provider, I should inform him/her about the nutritional protocol being provided by my Nutritionist, due to the potential of adverse reactions between Pharmaceutical medication and nutraceutical interventions.

It is important that I advise my nutritionist of any medical diagnosis, medication, herbal medicine supplements I am taking as this may affect my nutrition protocol.

I should contact my Nutritionist if I am unclear about any part of the nutritional therapy advised, meaning-Supplement doses, time of day or length of course of supplements.

Recording consultations using any form of electronic media is not permitted without written permission by my Nutritional Therapist.

The client notes the following-

Nutritional therapists are not are not permitted to diagnose or claim to treat medical conditions.

Nutritional advise is not a substitute for professional medical advise or treatment.

Each individual is unique, therefore there is no “one size fits all” and as such the degree of benefit obtained from nutritional protocol may vary from one person to another.
